Unique job opening as a Guardianship Case Manager, training offered for this multi-faceted position! Come join our energetic team where you can use and build upon skills in serving older adults in our community, advocate for client rights and make change to ensure vulnerable individuals are cared for. 

Full-time position with great benefits. We offer hybrid employment working both independently and as part of a dynamic 8-person team. 

A Sampling of What your Workday Would Look Like:

• Create and manage client centered plans of care and documentation
• Conducts client visits, to monitor status of clients living at home, in assisted living and long-term care facilities.
• Arranges for or provides services including but not limited to applying for government benefit programs, obtaining medical or health care, exploring/obtaining alternative living arrangements, assistance with housing problems and advocacy.
• Works collaboratively with outside professionals such as Elder Protective Services, legal services, community agencies, medical & personal care providers.
• Attend meetings in our Waltham office as required and be an active team member!

What can you bring to this position? 

• Stellar Communication Skills and willingness to learn
• Flexibility and initiative to manage crisis and on-going care management 
• Knowledge of community resources, entitlements, and health insurance 
• Bachelor's Degree with 3+ years related experience.  Master's degree a plus.
