Position Summary: 

Join our well-established Guardianship program as a Case Manager and vital part of our team! We are proud to be the largest EOEA (Executive Office of Elder Affairs) state funded guardianship program and we have been providing Guardianship Services for over 30 years!  This program serves our most vulnerable older adults many of whom have either been physically, psychologically, and/or financially abused by mistreated or are cases of severe self-neglect.

This is an opportunity to work independently while also learning from and contributing to a team of highly skilled professionals inside and outside the organization.  Using problem-solving skills, creativity, and interpersonal skills within a team environment we support our clients within our role as Guardian/Conservator as they navigate the challenges and transitions of aging through maximizing choice, dignity, and engagement.

This position offers a hybrid model of employment where some team meetings and other activities will require you to be in-person at our Waltham office while case management responsibilities would be in the field.  

We provide both individual and peer group supervision.  This is a full-time position with benefits. This position offers a $1k sign-on bonus. We work as a team to ensure client needs are met and decisions are made with clinical support.  We have a voluntary, paid on-call rotation for emergency after hours phone consults. 

During your workday, you will: 

  • Manage a caseload of clients.  Create and manage client centered plans of care as well as being at the ready when plans need to be modified. 
  • Conduct monthly visits, or more as required, to monitor status of clients living at home, in assisted living and long-term care facilities.
  • Arrange for or provides services including but not limited to applying for government benefit programs, obtaining medical or health care, exploring/obtaining alternative living arrangements, assistance with housing problems and advocacy.
  • Work collaboratively with outside professionals such as Elder Protective Services, legal services, community agencies, medical & personal care providers.
  • Consult with program's legal counsel and attends court hearings on an as-needed basis (training provided).
  • Document all work in electronic records in accordance with state guidelines
  • Participate in staff meetings, supervision, and training 

What can you bring to this position? 

  • Keen understanding of and experience with cognitive, physical, mental, and emotional capacities of older adults. 
  • Flexibility and initiative to manage crisis and on-going care management. 
  • Bachelor's Degree with 3+ years related experience.  Master's degree a plus. We offer supervision for licensure.
  • Knowledge of community resources, entitlements, and health insurance 

You will need a car and a safe driving record!  And the ability to effectively use Microsoft Office Suite.
