Position Summary:

The Associate Care Manager works under the supervision of or in conjunction with a Your Elder Experts (YEE) Care Manager to provide direct services to older adults in the Greater Boston Area. Associate Care Managers provide direct and collateral (indirect) care for YEE clients, under the supervision of or in conjunction with a Care Manager. You will actively engage in learning about all resources for older adults in the Boston Area. You will shadow care managers to gain greater understanding and working knowledge of the most important issues confronting our clients and their families.

Supervision Received:

Weekly and as needed supervision by director of Your Elder Experts/Care Manager Supervisor.

Specific duties and Responsibilities:

Under the supervision of a Care Manager:

• Take clients to appointments, on errands, and other places as needed
• Work with clients around their concrete needs
• Provide on-going connection with clients
• Facilitate complete communication paths between care manager, family and professionals involved
• Document efforts and time in electronic records

Requirements for Success:

An Associate Care Manager must be a dependable person with good common sense who feels comfortable working with older adults/people with chronic medical and or mental health issues in many settings including; their homes, doctors' offices, nursing homes and out in the community.  Experience working with older adults is a plus.  Candidate must have use of a car and a clear driving record.
